The Information We Collect and How We Use the Information

Registration Form

As part of the course registration process, The Society for Healthcare Epidemiology of America (SHEA), will require you to complete a registration form that will collect demographic information. This information can include information such as your name, address, title, organization/affiliation, work setting, age group, gender, race, and ethnicity. We collect this information soley for informational purposes to understand who we are catering to with our educational programs.

The information collected as part of these registration forms will not be shared or sold to any individual or organizations.

Payment Authorization Form

The payment authorization form will collect information such as your name, address, phone number, and payment information. Users can choose to pay by credit card, wire transfer, or check. If paying via credit card, users will enter credit card information through a third-party payment service that is 100% safe and secure. The information collected on this form will only be used to secure payment for educational courses in your cart.

Assessment Results

Any assessment results associated with any of our courses are accesible only to the person who takes the assessment and the SHEA LearningCE Administrator. In the case of the latter, SHEA will only collect quiz results, and not any personally indetifiable information, for the purposes of improving course content and associated assessments.

Course Evaluation Form

Users are required to fill out a course evaluation before claiming credit and printing/saving/downloading a certificate acknowleding the credit earned from the course. The form colleccts information about the user's experience with the course. The information collected on this form will be used internally to evaluate the effectiveness of the course and to guide changes and updates that are made. SHEA will not store personally identifiable information in association with the course evaluation.

Aggregate Log File Data

Log data is information automatically collected from the computers of site visitors. It can be viewed only in aggregate and does not include any information that is personally identifiable. Log dataincludes such things as the type of computer operating systems, the types of Internet browsers, and the name of the Internet Service Provider used by visitors to the site. Log data also includes quantitative information on which site pages and activities are visited most often. This information may be used to aid future site development by showing which features and information users access most.

Security

SHEA's Online Education Center, LearningCE, https://learningce.shea-online.org, maintains an information environment that protects the security, privacy, and integrity of all information collected and used within the site. Information is stored within server facilities that use the latest industry standards and practices for protecting information.
