C&C: When - And From Which Anatomic Site(s) - To Do Blood Cultures

Converse & Convene is a virtual networking forum moderated by SHEA member experts who will lead and facilitate discussion and collaboration. Participants will be able to share challenges, lessons learned, and gain greater insight not only with their breakout groups but with a larger group discussion directly following and lead by the moderators.
We will begin with an introduction of the topic before moving into breakout rooms. The conversation will focus on the following questions, but please feel free to bring your own questions, ideas, and stories.
- When diagnosing a central line bloodstream infection is there any role for obtaining a blood culture through a central venous catheter? If so, what are the best practices to minimize contamination and diagnostic ambiguity?
- Which pathogens should prompt follow-up blood cultures? In other words, when is it important to establish blood culture clearance?
- How can we balance the cost of blood cultures and utilization of healthcare resources with the clinical benefits of early and accurate infection diagnosis? What are the economic implications of different strategies for blood culture collection and diagnosis (e.g. Biofire BCID panels)?
- What are the most reliable clinical indicators and risk factors that should promote the collection of blood cultures in different populations (e.g. adults vs. pediatrics, solid organ transplant recipients, hematologic malignancy patients, ventricular assistive device patients)
- How does the timing of blood culture collection in relation to administration of antibiotics affect the sensitivity and specificity of pathogen detection and are there specific time windows post-antibiotic administration that still yield reliable culture results?
