Course Evaluation

INSTRUCTIONS:

All participants must complete the Course Evaluation in order to claim credit and print/download certificates. The deadline to do so is January 4, 2025. 
 
Participants can earn up to three (3) credits (AMA PRA Category 1™ Credits and/or ABIM MOC Points, CPE Credits, and/or CNE Credit) by attending all three live webinars. Please note that credit will not be offered for on-demand recordings; only participants who attended the live sessions are eligible to receive credit. 
 
If you are claiming ABIM MOC, please ensure your SHEA profile is updated with your correct date of birth and ABIM Diplomate number.
 
  1. Click the Access Course button.
  2. Complete the evaluation form and click the Submit button when finished.
  3. From the Course Complete page, click the Download Certificate link. You will first be prompted to enter the type and number of credits you're claiming. 
  4. Upon submitting your credit information, return to the Course Complete page to download your certificate(s).
